This file contains instructions to access the confidential data through the Secure data access center (CASD, https://www.casd.eu/en/) as well as the a do-file to produce the main results of the paper. Since the access to the data is confidential, we first describe the steps needed to access them. We then provide a simple replication file of the main results of the paper. Further assistance (and codes) in replicating our work requires access to the data and can be provided by the authors to other researchers that have access to them.
